Learning Objectives:
- Explain how the HEA Plus standards build upon HEA standards.
- Identify data requirements to identify populations and assess community social risks and individual social needs.
- Discuss the interconnections of social risks, social needs, service areas, community-based entities, partners, and stakeholders.
- Explain the roles and responsibilities of organizations and partners in delivering resources or providing interventions, as well as an accompanying Delegation Agreement, as appropriate.
